Patient messaging is another. Generative AI may help draft responses, but not every patient message is low risk. Some involve new symptoms, treatment questions, or issues that require direct Physician review.
These are not edge cases. They are examples of how implementation decisions shape safety, workflow, accountability, and cost. AI can improve clinical operations, but it can also create new forms of exposure when organizations treat implementation as primarily a technical task rather than a clinical and operational one.

Workflow is part of clinical risk management

Where does the tool sit? Who sees the output first? When is escalation required? What remains under direct Physician control?
Without clarity on these questions, variability increases. A tool may perform one way in development and another way in live clinical use because local workflows, staffing, and thresholds for action differ. This is especially important in higher-acuity settings, where small delays or misinterpretations can carry larger consequences.
Clinical risk mitigation now requires more than validation before deployment. It requires governance that continues after deployment and accounts for how AI performs in the real conditions of care.

Human feedback loops matter here. AI systems are used by people, and implementation should make it easy to identify what is working, what is not, and where course correction is needed.

That includes defining appropriate use cases, assigning accountability, monitoring outcomes after deployment, and making sure implementation does not deepen existing gaps in care quality, safety, or access.
